And they're perfect when you need immediate medical attention from a trusted provider and you can't visit your doctor's office in person. How to schedule your video visit
Log in to your online Providence Patient Portal. If you don’t have a Portal, click here to create one — it takes just a few minutes.
From your Portal on a computer
1. Click on the ‘Appointments’ tab
2. Click on ‘Secure Online Video’
3. Select the day for your desired appointment.
From your Portal on a smartphone
1. Go to the patient portal, and click on the ‘Appointments’ tab.
2. Click on ‘Schedule New Appointment’.
3. From the ‘Specialty’ menu, select ‘Family Medicine’ or, if applicable, ‘Pediatric Medicine’.
4. From the ‘Reason for visit’ menu, select ‘Secure Online Video Appointment’.
5. Choose from the available appointments to schedule with a Providence Medical Group provider. Note that the practice name and address is listed above the provider name.
How to begin your video visit
• After you've made your appointment, watch for an email or text that includes a link to the Providence Telemedicine web page or to download the 'Providence Medical TeleMedicine' mobile app. You can then set up your user profile.
• Before your appointment, log in to your video visit with your computer, tablet, or a smartphone such as an iPhone, iPad, or Android device — find the app 'Providence Medical TeleMedicine' on your mobile device, or click here from your computer. You'll also need a valid credit card.
• Please log in 15 minutes before your video visit and be prepared with questions for your doctor.
Tips for a great video visit
• Wi-Fi or a wired connection is strongly preferred for a great video visit experience.
• Use headphones — it cuts down on background noise and helps keep the conversation with your doctor private

Board Certified in family practice, Dr. Edward Hubach has been serving area families for over 35 years. He received his degree from the Philadelphia College of Osteopathic Medicine, and performed his internship locally at Grandview Hospital and Medical Center. He maintains hospital affiliations with Kettering Medical Center, Miami Valley Hospital, Grandview Hospital and Medical Center, Southview Hospital, and Sycamore Hospital.
Our professional staff includes Certified Family Nurse Practitioner Elissa Reed. Elissa provides complete clinical care, including treatment of illness and injuries, disease prevention, diagnosing and treating acute and chronic conditions, and annual wellness visits. Elissa also plays an important role in actively counseling our patients on ways to better manage their own health to help assure long-term wellness.
In addition to her FNP-C credentials, Elissa is also a Registered Nurse, with experience in trauma care and neurology. She is a native of Huber Heights, and looks forward to establishing caring relationships with families in the community where she grew up.

With over 10 years nursing experience, Dilya Makhmudova, APRN, NP-C,
offers our patients care and treatment for nearly every acute and chronic condition, including illness, injuries, infections, high blood pressure, diabetes, and many other critical health care issues. She also focuses on disease prevention, personalized health education and wellness counseling.
‘I believe that compassion, empathy and care can make a difference in people’s lives, and it’s very rewarding to see my patients’ health improving through education and support. I do my best to go above and beyond to deliver quality care to my patients.’
Dilya has extensive experience in case management, so she can help our higher risk patients manage care across the full spectrum of health services. She is also multi-lingual, speaking English, Turkish and Russian fluently.
As a certified family nurse practitioner and a vital member of our care team, April Lenz, CNP, provides treatment of illness and injuries, routine check-ups, and diagnosing and treating acute and chronic conditions. She also focuses on disease prevention as well as personalized health education and counseling — she looks forward to helping guide our patients to better health and wellness through every stage of life.
In addition to over a decade of nursing and care coordination experience, April also has an extensive background as a licensed social worker. She looks forward to using her knowledge and experience in helping guide our patients to better health and wellness through every stage of life!
We’re proud to welcome Certified Family Nurse Practitioner Kim Inman to Providence Medical Group – The Heights! Kim joins our professional care team in offering complete, compassionate health care for everyone in your family, from infants to the elderly. As a primary care specialist, Kim offers wellness visits and disease prevention education, as well as treatment for illness, injuries, acute and chronic conditions.
She also counsels our patients on ways to better manage their own health and to support their well-being — “Getting to know my patients, and the many things that impact their health, is so important in caring for them completely and over the long term. Helping each person achieve wellness, rather than just treating their symptoms, is my ultimate goal.”
In addition to her nurse practitioner credentials, Kim brings an extensive clinical background to our staff, including years of experience with intensive care, trauma care, nephrology and pulmonary patients. A long-time Dayton area resident, Kim earned her Bachelor of Science, Nursing degree from the Chamberlain College of Nursing, and her Master of Science in Nursing, Family Nurse Practitioner degree from Ohio University.

We’re proud to welcome Certified Family Nurse Practitioner Shannon Graham to our practice! As a key member of our clinical team, Shannon offers personalized health care for people of all ages, including wellness visits, treatment of illness and injuries, and care for chronic patients. She also focuses on disease prevention as well as health education and wellness counseling.
Shannon looks forward to building pathways to better health for each person. “Treating each patient as a unique individual is so important. Taking into account their health history and everything that impacts their well-being — seeing the whole picture — is vital in helping them achieve better health and quality of life.”
Shannon brings an extensive clinical background to our staff, including over six years experience in emergency medicine. She earned her Bachelor of Science, Nursing degree from Kettering College, and her Master of Science in Nursing, Family Nurse Practitioner degree from Cedarville University.
A Dayton native, Shannon enjoys spending time with her family and participating in both community and nursing organization activities.

We measure our progress by tracking the care and treatment of groups of patients who are at risk for common or chronic conditions — here are some important at-risk factors that we carefully monitor:
Blood Pressure Control
Patients 18 to 59 years of age with hypertension whose blood pressure is less than 140/90.
2018: 63% 2017: 59%
Diabetes: Controlling Cholesterol
Patients 18 to 75 years of age with diabetes whose most recent LDL-C value is less than 130 mg/dl.
2018: 77% 2017: 75%
Colorectal Cancer Screening
Patients 51 to 75 years of age who have received one or more screenings for colorectal cancer.
