It is with mixed emotions that I am announcing my retirement as of December 31, 2018. It has been a pleasure taking care of your health care needs over the many years and it is not easy to give it up. All of you seem like family after all of these years, and I hope that feeling will not change as time goes on even when there is no formal bond between us as of the above date.
I am happy to announce that Autumn Conde, DO, will be joining Providence Medical Group and will be taking over my practice as of January 1, 2019. Dr. Conde is a former Centerville resident and graduate of Ohio University College of Osteopathic Medicine and will be returning from Oregon after practicing there for eight years.
Additionally, my dedicated office staff will remain and continue to support your needs in their usual friendly and competent ways. Your medical records will be available in my office and the transition should be uneventful. However, if you wish to transfer to another health care provider they will be released upon your written consent.
Again let me wish you a healthy life and thank you for your confidence and friendship over the many years.

Board Certified in family practice, Dr. Autumn Conde provides complete primary care and wellness services for patients of all ages. She takes an active interest in the well-being of every patient, with a view toward continuity of care for each family member to help assure on-going health.
Our practice welcomes new patients and accepts most major insurance plans.
Anthony Lugo, MD, welcomes Autumn Conde, DO, to his professional staff. An experienced, board certified family physician, Dr. Conde will be seeing Dr. Lugo’s patients as he retires from family practice.
Dr. Conde works in partnership with each patient to achieve better health and wellness. “I believe in my osteopathic principles to look at a patient as a whole and not just a single problem. To the best of my abilities, my goal is to treat and counsel each patient with care and compassion, as if they were my own family.”
An Ohio native, Dr. Conde is an honors graduate of Wright State University, and received her medical degree, also with honors, at the Ohio University College of Osteopathic Medicine. She also has an extensive background in emergency medicine, with certifications in basic, cardiac, trauma, pediatric and neonatal life support.
Of special note, Dr. Conde has devoted much of her career to helping underserved populations in the United States where primary care is difficult to access, and, with a subsidiary of Doctors Without Borders, treating people in areas of Africa and Nepal where a doctor’s care is all but non-existent.
