Welcome to Germantown Family Medicine, where certified nurse practitioner Krista Miller provides compassionate, personalized health and wellness care to patients of all ages.
In addition to patient-centered primary care, we specialize in preventive medicine and women’s wellness. New patients are always welcome; most major insurance plans are accepted.
With an approach that stresses both care and cure, Certified Nurse Practitioner Krista Miller provides treatment of illness and injuries, disease prevention, diagnosing and treating acute and chronic conditions, and annual wellness visits.
As a nurse practitioner, Krista also plays an important role in counseling our patients on ways to better manage their own health to help assure long-term wellness.

Blood Pressure Control
Patients 18 to 59 years of age with hypertension whose blood pressure is less than 140/90.
2018: 96% 2017: 96%
Diabetes: Controlling Cholesterol
Patients 18 to 75 years of age with diabetes whose most recent LDL-C value is less than 130 mg/dl.
2018: 87% 2017: 93%
Patients 18 to 64 years of age who received an influenza vaccination during the measurement period.
2018: 47% 2017: 52%
